What is the Vial of Life?

In a Medical Emergency, thinking clearly is difficult; critical information regarding a patient is not passed on to pre-hospital care providers and in turn the receiving ER and hospital staff. Such an oversight can be life threatening. The Vial of Life program was conceived to help alleviate this situation.

By recording your doctor's name, important medical conditions with a list of medication and placing them in a canister in your refrigerator; trained medical responders will be better equipped to help in an emergency.

 

Request for Public Records

Chief Lipe, Public Records Officer under RCW 42.56.580.  Request for public disclosure may be made during the normal business hours of the District or on the district's website, per resolution 08-297.

Upon completion you may drop it off at VIFR's Business Office  (10020 SW Bank Rd), fax (206.463.2954) or mail (PO Box 164) your  request. 

Local agencies are required, wthin five days of receiving a public request to respond by (1) provide the request  (2) acknowledging receipt of the request and providing a reasonable esitmate of the time required to fill  the request (3) deny the request.  There is a nominal fee of .15 per sheet for photocopies. (RCW 42.56)
